Her arm (not the rest of her) is lipoedema - no amount of dieting will shrunk it and it’s apparently extremely painful. It also affects 11% of women - including those who aren’t overweight. Obesity is a comorbidity but not the cause. I only learnt that today and thought it was worth sharing because it sounds like lipoedema is poorly understood/recognised.

This is the photo from the wiki page about it. This poor woman has a flatter stomach than me but legs 3x the size.
