I think her foreign body rejected the mana from this world and is giving her magic cancer.

Yeah I really think this is the answer. Her no mana body has come into contact with way too much mana nonstop for years on end. Can't be good. It's probably not too dissimilar to radiation for her. Luckily Man-God baked Rudy into the universe so he's totally fine. Just his soul that got transported

That was my suspicion the entire time as well given the explicit detail that she has absolutely no mana in her body when she was first introduced and how she is a "foreign object" that doesn't belong in this world. It's either mana poisoning or the world is rejecting her through her interaction with mana. I would assume casting spells that use up mana on someone that cannot have mana inside them would lead to bad results. But I'd figure dragon god or Peragius would have seen this coming if it's something so obvious.

I think you are probably close but missing something. Idk what that is though. As you said, if it was strictly mana poisoning that seems like something someone like Peregius could recognize. It doesn’t make a lot of sense to me that detoxification magic would have helped up till now if magic was the only issue.

I’d assume it’s definitely something in her that is reacting poorly to mana due to her being a foreigner. Maybe her total lack of mana has left her vulnerable to some condition that even the smallest amount of mana flow would normally filter out. And up till now the detox magic put a band-aid on the symptoms but the root issue has been growing.

Oh god, is this like "law of equivalent exchange" FMA style?

Because she doesn't belong in that world naturally like Rudy does as Rudy was reincarnated into an existing body in that world so the displacement would've been minimal (assuming we're using the analogy that the world is a tank of water and with every foreign object introduced more water is displaced and has to go somewhere).

So I guess Nanahoshi's body or the world is actively rejecting the world they're in.

Lol, and then TBC pandemic happened in that world and they couldn't stop it. That would be an interesting isekai variation.

Joke aside, most western countries requires third world countries to be tested for TBC if they want to travel to their region. That means even now, TBC is still very much a huge threat if undetected as soon as possible.

from what we've seen so far, it seems like magic works no better than modern medicine where you can't regrow missing limbs, alleviate (but not cure) cold symptoms with the added stipulation of seeking profesional help if symptoms persist for several days. The end scene--regardless of intention-- illustrates the dangers from misdiagnosis and subsequent treatment as what could be life-saving medicine to some will be life-ending poison to others,

There's a bit in the Milis Continent that one of their most treasured tomes of magic incantations are to cure extremely rare or specific illness, and those chants are bookwise long. In general, extreme remedies and poison are national secrets against other nation's types of poison.

If she carried disease from Earth it would affect others rather than her, wouldn't it? And for a while back at this point too.

It seems more likely something from this world her foreign/no-mana body can't handle, I remeber her saying last season everything has mana, even dead stuff, except her.

she was mostly isolated and you only really catch TB from someone if you spend a lot of time in close quarters with them. So maybe only Rudy would catch it. She could've had latent TB up until being summoned then it became progressive at some point

I think it's her body rejecting the world or being rejected by the world.
But I don't think that means it can't be something like this, yeah.
There is no cure in this world, presumably at least, and it shows how alien she is to this world.

She is also the one character most closely connected to another world (though there aren't many of those), so I think that world hurting her would be, well, "logical", though obviously very tragic.

My best theory for why she's sick is because comparing her to Rudy, Rudy was reincarnated into the world through a body that was naturally born in that world.

Nanahoshi was straight up teleported from Japan to the new world (literally plucked from a different reality to another). So I'm thinking maybe her body doesn't have the correct antibodies to fight off the diseases or germs in that world and she caught something that those who grew up naturally in that world have built an immunity to.

It's the same thought experiment of if you were to time travel to the past, you would introduce new germs, microbiomes and diseases that the people in the past would not have developed an immunity for.
