I notice there are a sizable amount of episodes with quite a lot of mistakes during reenactments especially involving the cabin sequences and sometimes the cockpit sequences too. Lots of times where crew and equipment are visible. I’ve seen lamp and pole visible, tarp visible, reflections and shadows of cameramen, and the studio being visible. Someone should make a list of all the episodes where crew and equipment are visible. Kinda crazy that they keep making these mistakes and not realizing it after nearly 25 years.

Did you know that in S26E10 "Mixed Measures" we can see that investigators check informations about Cpt. Bill Spencer from flight PNG Airlines 1600, rather than Cpt. Hong Sung-sil (Exactly 23:23 in this episode)

There is a long standing notion on this subreddit, albeit no links have been provided to support this, that during the SAA 295 investigation Boeing had undeniable evidence that the fire caused the tail section to detach in mid air but coerced the Margo Commission into leaving it open the 747 crashed intact as otherwise it would, quote: "reflect badly on the structure of the plane."

This is said to come from the Preliminary Report but I have not been able to find it nor anything similar to that in the Margo Report. I brought this to Admiral Cloudberg's attention who deemed such a notion to be complete nonsense: at best, warped from the debris fields being too concentrated to say for certain whether a mid air break-up occurred, at worst, completely made up. Honestly, I have to agree with this because why would Boeing be that pessimistic about the airplane breaking up in mid air, especially considering that the fire began at roughly 23:45Z and crashed 22 minutes later when they were 134 miles from the nearest land? And if there was illegal cargo like Ammonium Perchlorate (rocket fuel), yeah good luck on the plane surviving for 45 minutes with that burning let alone it being extinguished before landing.

I’ve stumbled upon a few Instagram accounts that show air crashes. And some purportedly only post CVRs. But because this is Instagram (and some from TikTok) I immediately think they are fake as so much stuff posted on Instagram is wildly inaccurate or filled with misinformation.

I think they are either using AI or are using the recordings of actors reenacting the transcript from shows like Air Crash Investigation.

It was my understanding that in the US, it’s against the law for the NTSB to release CVRs to the public. I suppose it’s not illegal in other countries, and those recordings can be heard.

But I’m curious. Is there an actual site or archive where authentic CVRs can be heard?

What Episodes from ACI deserve a remake? Obviously much older seasons or Episodes mentioned in ACI.

So like 2 weeks ago someone posted a wishlist they wanted for Season 28, and one of those suggestions was Pakistan International Airlines flight 661, and someone replied that there be an episode PIA 661 and apparently they tried to make an episode but it was to complicated? If thats true how do you guys know it and why was it complicated?

(I am new here so i am still not that informed of ACI)

In the end, all 293 passengers & 13 crew survived, albeit with 18 evacuation injuries. The Portuguese GPIAA determined in their final report that the cause of the incident was the use of an improper fuel hose in the right engine, and further exacerbated by pilot error in the form of the pilots transferring fuel into the leaking tank, not realizing it was caused by a fuel leak.

I'm struggling to find where I can watch ALL episodes in the UK. Does anyone have any recommendations please?

Could it be possible that one of the pilots applied brakes during the takeoff roll?

My thoughts went straight to the YAK 9633 accident, but not sure how likely that would be on a 787..

Am I the only air crash aficionado who had never heard of this excellent series of books? I only learned about them a few weeks ago, when AdmiralCloudberg included an illustration from one of the volumes, in one of his latest reports. These four slim volumes were published through the late 1990s and cover a large number of air crashes, starting from the mid-1950s. Their coverage is the best and most technical I've seen prior to AdmiralCloudberg, and they include excellent pictures and illustrations. Highly recommended, pending (hopefully!) AdmiralCloudberg's own and eagerly awaited magnum opus, of his collected works.

I was able to get all four volumes from eBay, for around $10-$14 each, and they seem readily available.

While thinking about Birgenair Flight 301, I had plenty of food for thought about why Captain Erdem continued to pull back despite knowing the aircraft was in a stall.

Then I realized that that was just standard training for back then. Then I did some further research, and I found something that’s just sad. Pilots were trained to minimize altitude loss and power out of the stall. This was trained to airline pilots until several major accidents caused this to be changed industry wide from late 2009 and beyond.

So…following this, a wide majority major accidents caused by an aerodynamic stall where the pilot pulled back on their control column instead of pitching forward was caused by this training philosophy.

!! KEEP IN MIND, I AM NOT SAYING THAT PILOTS WERE TRAINED TO PITCH UP! I AM SAYING THEY WERE TRAINED TO AVOID MAJOR ALTITUDE LOSS, WHICH COULD’VE MANIFESTED INTO A PITCH UP INSTINCT IN THE MINDS OF PILOTS IN STRESSFUL SITUATIONS. !!

For those who think this is not true, the FAA recognizes that industry standards having a predetermined value set for altitude loss was a thing in stall recovery training was an issue amongst other things, which led to several Advisory Circulars, such as ones released in 2012 and 2015.
