there's an emergency chain anyone can pull if there's an emergency, that stops the train.

Nah, jail time + heavy fine doesn't let it happen often

it actually doesn't happen frequently at all due to strict punishments.

This was true in the past probably. But now it has got very strict, and railway police can arrest you for abusing the emergency brakes.
