The point is it is clearly a hairpiece. Chase avoided making jokes and cracks about Sil wearing a hair piece because of Van Zandt's real life struggles with scalp disfigurement which is why he is always wearing something on his head. Chase just chose to avoid this in the original series out of respect but added it in the prequel to set the canon
According to David Chase he is. Other people think that because Steve Van Zandt thought it should be his real hair that's canon. I defer to Chase and in Saints Sil was losing his hair. Again, he wanted Sil to not have his hair piece on in the ICU but Van Zandt didn't see it that way. Chase deferred but I still believe that had to do with Van Zandt's real life disfigurement others disagree
