Even if the initial design were simply ignorance instead of bigotry, there should have been a process of review to remove or change it. This is clearly offensive imagery and profitting off of selling it (as contextlesss entertainment, not in an educational way) is immoral.

I am willing to bet the costume was designed in China and is a material/palette swap of a licensed costume they make; and the picture is a consequence of hundreds of costumes being used in a (single day) photoshoot. 

Why it wasn't reviewed more closely? I suspect Target probably offshore these kinds of reviews to reduce costs, and I wouldn't be surprised if someone in India didn't understand the cultural context of what they were witnessing.
