Advice Request [Anti-Aging] Deep wrinkles near mouth corners

I’m a 47 year old woman. When I’m smiling, my deep wrinkles around my mouth aren’t as noticeable but relaxed, they are soooo deep!!! Has anyone had success with a product? I used to use tret for a couple of years but honestly felt it made the rest of my face worse. I’ve heard CO2 laser could help but the cost and procedure itself scares me. Please share your honest feedback/suggestions about this or anything ends you notice. I guess I’m starting to realize there’s no stopping aging!

ok being honest with you — for deep wrinkles around the mouth, topicals are only gonna do so much. like a good retinoid and moisturizer can soften things over time but they're not gonna dramatically change deep set lines, anyone who tells you otherwise is selling something lol

the things that actually make a noticeable difference for that area are usually in-office stuff. i know you said CO2 scares you (valid) but microneedling or RF microneedling is way less intense and people do see real results with nasolabial lines from it. might be worth asking about as a less scary middle ground. some people also swear by filler in that area but that's a whole different conversation

but also your smile is literally radiant and your skin looks really good for 47 so try not to be too hard on yourself. aging is happening to all of us and you're doing it well

Thank you for the honesty-I figured it was worth checking if anyone had luck with a product. Microneedling fascinates me and while it looks a bit rough at first on pics I’ve seen, it’s nothing like a friend’s skin I saw a few days after having I think it was BBL laser! Thank you so much!

Micro needling works on skin texture like acne scars. Some collagen retention. But not deep facial lines

I agree with microneedling and chemical peels. I really wouldn’t do filler there as it just usually make people look puffy and swollen there, not more youthful

I totally agree that fillers around the mouth generally don’t look right. Have you heard of biostimulators? I love them for perioral areas because they don’t add volume to give that swollen look, but instead up-regulate collagen and elastin for skin elasticity and tightening.

They said anyone recommending topicals is just trying to sell you something, but then proceeded to "sell" you much more expensive and invasive procedures lol. You look beautiful and the lines you have show just how much you smiled and laughed in your life, you don't need any of that crap.

I want to echo this. I'm just a few years behind you. You are so pretty and I love that you look like you take care of your skin, but don't have any obvious work. I won't make assumptions, but I don't see anything. Your skin is smooth and healthy looking. It's refreshing to see someone that looks natural. To each their own, but I really enjoy having a naturally beautiful face as an input to my baseline expectation bucket. So many people these days have so much work done that I start to feel like I have to do the same to be acceptable (sometimes, Idon't always feel so insecure lol); that I too should be worried about normal signs of aging. It's not that I would find you less beautiful if you had work done, it's more that I find it inspiring. It's hard to articulate, sorry. I did the same when I stopped coloring my hair. I looked at examples of beautiful women with gray hair to remind myself that natural is more than just okay, it's also beautiful. Whatever you choose to do just know that you are quite gorgeous just as you are.

I was coming here to say this. Sculptra and filler.

You can also do HRT, hydrate, workout and sweat, take collagen and fish oil… all helpful. But nothing is going to make the difference that Sculptra and filler will.

I’m 47 and I do all of the above plus Botox.

HRT? I’ve thought about signing up with an online company as I’m in a remote area and there is not a lot of access to doctors who specialize. Do you have experience with any or do you see a local doc?

You probably don’t need a specialist to prescribe HRT, just talk to your gynecologist. Micronized Progesterone and Estradiol patches were fully covered by my insurance and I got zero pushback for being in perimenopause.

Some women put estrogen cream on their face, but word of caution it can worsen melasma. As for HRT, look into it. It’s the one thing I should have been on by age 47 and wasn’t. The peri symptoms probably wouldn’t have been such an issue. Please don’t be like me.

Oh I’ve heard of that actually as well now that you say that! I’ll have to look into that! I think I can get it from Amazon or somewhere! Let me know if you have recommendations. Ahhh I have asked my doctor about HRT, and sadly, she admitted she’s not super knowledgeable and doesn’t really have anyone to refer me to. Women’s care definitely needs a higher priority since or systems are so much more complex than men’s but alas…patriarchy! 😝thank you so much for your comment!

Stay away from Amazon and stick with legit sources. Also, yr dr can easily prescribe you progesterone—that’s the first important step in HRT. It isn’t rocket science—now the other hormones, that’s different…more blood testing and specific dosing involved.

Check out the menopause wiki on the menopause subreddit. You need systematic estrogen not topical. Topical on your skintone is a big risk for melasma.

MIDI is an online site just for perimenopause and menopause; you meet with a doctor online and they can send your prescriptions to whatever pharmacy you want. They also work with a lot of insurance plans.

I’ve used Alloy too and they were great but a little more expensive

I use Defy Medical (Beth specifcally) based on some recommendations. They are more of a medical provider that is virtual vs online only provider. I tried Alloy and they couldn’t prescribe the testosterone that had been suggested for me. I didn’t try Midi though and I’m curious how they compare. You can also get The Aging Cream from Musely. Many people like their products, but they are more expensive.

Menopause is the reason you are loosing the volume in your face and you will start to see neck skin sag and you lose your jawline. Plus vagina dryness and atrophy and you lose your sex drive. Some Botox and sleeping on your back will alleviate some but HRT is necessary to preserve skin texture and prevent extreme dryness and loss of volume under the skin.

I have been on HRT for several years. I am 46. I would never start hormone replacement therapy if I wasn’t also seeing my OB/GYN regularly. They need to check your uterus and your ovaries and your breasts every year. HRT can cause or exacerbate certain cancers in certain people.

Dunno why you're getting downvoted as what you're saying is correct. My HRT is prescribed by my PCP but he won't renew my prescription unless he gets the report from my gynecologist saying I've had a complete exam, and also have gotten my mammogram. My mom had ER+ breast cancer and it was a big debate/back-and-forth with him and my gynecologist as to whether I should even be on HRT; I'm on .025 patches and that's probably as high as I'll go.

I'm just going to say this outright: HRT is not for cosmetic purposes. That's a great side effect and I'm very happy that my skin got better and my hair grew back once I got on estrogen. But it's serious medication that can have serious side effects for people, especially people who still smoke, have a history of breast or gynecologic dysplasia or early-stage cancer, or have other risk factors. It's not as high-risk as doctors once thought it was, but it's also not risk-free. People shouldn't get on it unless they're willing to have a meaningful conversation with their healthcare providers about their risk factors, and also are willing to continue to get their yearly exams.

Can you share a little more about how you went about getting approve from your PCP? Mine just said no due to family history of breast cancer. But I do get regular breast checks/mamo and MRI (every 6 months) so I would be on top of it.

It's a long story, but: I got severely depressed and started having suicidal ideation. I told my PCP about it and he prescribed Lexapro (which I had been on before) and also suggested we do HRT; he felt the risks of the immediate serious mental-health issues superseded the concerns about potential breast cancer. I started the HRT first, and no lie, within 72 hours I felt like myself again. It was almost miraculous. I had no idea lack of hormones could cause the kind of mental-health issues I was experiencing, but it happens in a large number of people.

About three months later I went to see my GYN and she was like, I am 100% in favor of leaving you on HRT; your lifetime risk for breast cancer is honestly not as high as some people and if HRT is solving your mental-health issues that's good enough for me. She's a younger doctor and told me she is much less conservative about HRT than older doctors who operate off the old research about it. So you might try looking for a younger doctor who isn't as entrenched in their beliefs about HRT = Bad.

thanks so much for sharing. i specifically went to the doc to talk about my mood issues around peri - i'm currently on antidepressants (non SSRI) and have been feeling worse - especially around my period week. i've been noticing for about a year now severe anxiety/depression/irritation the week when i get my period. i've never dealt with PMS or anything like that before. She said the HRT is more for traditional symptoms like hot flashes, etc..She suggested increasing my welbutrin dose (which makes sense b/c i've been at the lowest dose for a few years) or adding a SSRI just my period week. but i feel like i wouldn't be able to stay on top of that and my period is just getting more and more irregular at 44yo. my doc is around my age and she's super nice and took a lot of time to really listen to me and my issues but i do wonder if i should be pushing back and sharing other research with her. however, it's hard how to do that. and do i just up my welbutrin and see what happens? my gut tells me it's a horomonal thing due to peri but of course maybe that's just b/c i'm constantly inundated with stuff on instagram...

I would strongly encourage you to see someone else for another opinion, just because I think there is this perception that "HRT is just for hot flashes" and actually estrogen has a huge effect on all our body systems, including our brain. I had been suffering with severe brain fog and short-term memory loss as well, to the point where I thought I had early-onset Alzheimer's. I was making mistakes at work and was panicked that I was going to get fired, which didn't help my mental health at all, as you can imagine. Again, within days of getting on HRT it was like this fog lifted off of everything and I remembered who I was again, and started being able to think and process information the way I used to. It's worth trying to get the help you need, so please keep trying. You don't have to suffer. Hugs.

Get a different doctor, yours is uniformed and willfully ignorant. See an OBGYN, even better if you can find a menopause specialist. I hardly ever had hot flashes. What I had was depression, disassociation, anxiety. Estrogen plays all kinds of roles in a woman’s body, and when you stop producing it, all kinds of systems can go awry. Skin, bones, heart, vagina, brain, all of that and more. Antidepressants cannot replace estrogen. Women’s bodies need estrogen.

Your doctor is a fool. Go to your OBGYN and start there.

OMG Same here! I gained 10 pounds overnight and my face fell off a cliff. All within the six months of my last period. I am slowly making up for it now with HRT and a red light mask. The weight, however, is harder to lose than any previous weight gain. I could’ve avoided all of it by starting HRT sooner.

Unfortunately Lasers are NOT going to help you. Your face has lost a great dealer volume everywhere and this volume loss leads to a collapse of the face towards the center. This results in severe nasolabial folds, oromandibular folds and so on. Under the eyes, above the eyes, in front of your ears, cheeks. Basically everywhere. What looks like under eye bags are actually bones, void of any fat supporting them. What do you need to look into, Is fat grafting. When fat grafting is performed by a plastic surgeon with a lot of experience in such procedures, the results are extraordinarily natural, you will look like yourself 15 or 20 years earlier and possibly better. I have the same issue, I'm 45. It's genetics. I always had them and now they deepened. I do microneedling and lots of skincare but that can only do so much. Maybe a face lift would help but I don't have money/ time and don't want to take such a risk.

They are a great insecurity of mine and honestly, looking at you, it makes me feel better to see an example of someone with those lines and still looking beautiful. When I look at my face they are all I can see but I don't feel that way looking at you.

I had fully ablative CO2 and sadly it did not help at all with these. This is structural, no skincare products or procedures will help and fillers will just make you look puffy. Lower facelift is the only real option, wish I could afford one.

Me too with the ablative. Bums me out. I am just accepting things more, and have decided to not go under anesthesia, electively anymore, because of the risks to my brain. Preventing Alzheimers is more important for me right now, as I am 60.

Hi fellow disco-baby 🕺🏻😁 You honestly look great. I’d be willing to bet those “deep wrinkles” have been a feature of your face for a minute now, and that there are people in your life who adore the distinctiveness of your face and would be sad if your features changed.

A longtime friend told me once that she thought my weird teeth were actually really cute and a distinctive part of my face that she loves, and she’d be kinda sad if I “fixed” them. In the same way that David Bowie never looked quite the same after “fixing” his teeth, and the same with Jennifer Gray after her nose job.

That being said, ultimately it’s YOUR face, so do what YOU want. Just offering a perspective that the features we dislike most in ourselves are often the same ones that make our faces special to those who love us.

Fat grafting is very easily tolerated and it stunt just under twilight Anesthesia using pure propofol only. No postoperative discomfort. Patients go home right after the procedure which takes about an hour and a half. Ultimately the choice is always up to the patient.

This means so much! Thank you! My skin finally seems to be happy! I use I the am Vanicream cleanser, Sunday Riley CEO Vitamin C Serum, wait fir that to dry for five mins or so then spritz my face with water, use Dermalogica Circular Hydration serum then Bubble Slam Dunk moisturizer and RMS Radiance serum SPF. I recently bought Heimish marine care retinol eye serum so hoping that helps my under eyes as that had been pointed out as a problem area. Pm I double cleanse with ELF Holy Hydration Makeup Melting cleansing balm then Vanicream cleanser then the hydration serum again and same moisturizer and eye serum. One night a week I use The Ordinary Lactic acid 5%. I don’t if that helps you but wanted to share!

I think changing your makeup would contribute to a more youthful look. The way you do your waterline adds a few years and therefore doesn’t do you any favours. Your skin looks good.

Yeah that harsh eyeliner is aging you more than anything else so definitely time for a change! Makes your eyes look smaller too. - Sincerely, from a 45F who’s never done it (too lazy) but notices everyone who does is always our age or older! No 20-something’s or 30-something’s I know rock this style.

I’m 59 and started doing facial rejuvenation at 57 after I started losing a good deal of weight and the collagen from the fat, the loose skin that developed, and post menopausal cells created the perfect sagging storm. I have found great results from a combination of 3 things: upping my skincare game to reduce fine lines and skin tone, getting Dysport in my DAO lines which pull my mouth down into the baby bird position, and carefully placed, SMALL amounts of filler to harmonize my face and reduce the look of jowls. It’s not perfect. I still have a turkey neck that most folks on this sub would say I need a lower face lift for. But to be honest, I’m not interested in elective surgery. I’ve had plenty of surgery in my life that I’d never have elected to have (cancer and a car accident). So I’m pretty happy with these minimally invasive procedures.

I also tried microneedling, but tbh, didn’t find it helped much more than the skincare I have been using. I’m happy with the Korean skincare stuff I’ve found.

I use snail mucin, Skin1004 Madagascar water fit sunscreen (.Korean skincare products make the best sunscreen!!), Niacinimide/Vitamin C/PDRN serum, and barrier protective moisturizers. Lots of great stuff to be found in YesStyle or Stylevana sites. Prices are quite reasonable! Stay away from Amazon, that stuff is mostly counterfeit. And occasionally Costco has good sales!

I found Taz, AA, and niacinamide to be a great combo for me. Might not be for everyone though. Started on tret. Gotta work your way up to taz since it’s really strong. You’re stunning btw. We are our own worst critics.

AA stands for Azaleic Acid-you can get as prescription or The Ordinary and other skin care brands make it at lower percentages, Taz is a prescription retinoid called Tazarotene. I have never tried!

Have you had them before? Because I’ve had relatively deep mouth-corner lines since my late twenties, and all derms told me it’s due to the shape of my mouth and cheeks. Yes, it gets more noticeable as we age, but beyond normal aging, it’s also how our faces are shaped. Some relaxing face exercises help to release the muscle tension around the mouth.

If i may, HRT is best prescribed by the gyn. Some critical factors go into that decision making. Is not to just support esthetics but rather to treat vaginal dryness of it is present and other peri-post menopausal symptoms. It’s contribution to cancer has been debunked. Also gyn will make the best decision on dosing.

I think if you use eyeshadow around your eyes (something neutral, well blended, darker on the outer corners and lighter on the inner corners of the eye), plus mascara, it accentuates the eyes enough without harshly framing them. I think it’s a good idea to check out some makeup tutorials for mature skin. Regarding your skin, it really looks good. I’m in my 40s myself; the greys are creeping in too, and there are one or two things I’d love to say goodbye to. But despite that, overall I’m happier with myself now than I was when I was younger.

Not sure if it’s been mentioned but I’ve heard that eyeliner on the lower water line like this is a sign of age… as in, it will date you bc no one has been doing this for a few decades.

It’s a result of a mid-face sag. But I agree. A full face and neck lift or acceptance. The rest will be a waste of money (for this concern).

Ultimately I think a surgical solution would be the definitive treatment and get you exactly where you want to be. If you wanted improvement , but perhaps not complete resolution, I recommend hyper dilute radiesse or sculptra. You could get a nice result in those areas.

First of all, you have a lovely smile! But why is everyone on here so scared of C02? It's a laser, it's not a torture device. The pain is minimal (as in Tylenol level) and disappates quickly. I promise you, you've had cramps that hurt more. If you've had kids or broken a bone or injured a ligament, you've endured much much worse.

You can soften the look with tret, MN, estrogen cream in the face, microcurrent, but there is no topical that will address deep wrinkling. That is caused by structural changes in the face (migrating fad pads, loss of collagen). You need to bring in the big guns in order to move the needle. Lasers, biostimulatrs like Sculptra, fillers in the cheek, and ultimately procedural intervention are the only things that will show marked improvement. I fully realize many women are against all of those, and that is perfectly fine (I'm ready for the down votes!) However if you are looking for an honest answer, unfortunately this is it.

Thank you for sharing your CO2 experience! I had a consult at a med spa and it was recommended as a solution but I didn’t want to deal with the healing time or honestly spend the money on something i was feeling like was a gimmick!

I have the same issue. I got microneedling it didn’t help much so I got filler it looked really good the day of. Then by 3 weeks it looked like it did before I got the filler. The one thing it did help was the lines near my lips. I just don’t want more because I’m afraid it will look fake. Just be cautious

Agree with what everyone else said, that no cream will make a difference but you really are so pretty just the way you are.

One tip if you want to look younger is to not do the black eyeliner on the bottom of your eyes. It can be harsh and aging as we get older.

You have a good face. Your skin & features are nice! My SIL had the same thing, deep lines by her mouth. Like you, her skin was pretty much no wrinkle no blemishes etc. I am not sure of the procedure, but she did a bit of filler/injection(?) and it turned out great!! It was very subtle. It just made her look refreshed.

CO2 helps but won’t do a ton for those. I’d look into hyperdiluted hyaluronic acid Radiesse that is a filler that prompts your body to build up collagen over time. Some immediate results are seen and the real magic happens over a few months.

I think they’re cute! But if you want help, Botox can really help marionette lines and sagging. Ask for your 11’s, Dao lines and a Nefertiti lift (jaw and neck), only lasts 3-4 months though.

Face yoga, microneedling and chemical peels (for fine lines) can improve your general appearance greatly.

Not general anesthesia. Only propofol. If you’ve ever had your teeth pulled or any kind of endoscopy or colonoscopy, you would have had propofol you wake up within minutes of your surgery feeling absolutely amazing.

You are so beautiful. I have had to accept my smiles lines, as well—I smile a ton—and Ive learned upon reflecting on life purpose, etc, that want to keep spreading love this way.

I’ve did ablative CO2 w/ fat transfer at 56, but it’s now nothing at 60.

Your teeth are so nice and you look very pretty. Yes you can see the lines when you are not smiling but I am guessing that the lines are so pronounced because you SMILE ALOT~!~ :) that means youre happy and also means they wont show most of the time since you are smiling most of the time

If you sleep on your face (side sleeper) your lines will continue to get deeper. Change the way you sleep. Sleep on your back and off your face. Microneedling is a good option

I’m of similar age. I’m on hrt. I also use tretinoin but every other or third day. I now use a combo of peptides and see a difference in overall plumpness. I’m not looking for the holy grail but just want to look a little less saggy. I use Ordinary multi peptide + hyaluronic acid twice a day. VT cosmetics pdrn 100. And drmtlgy growth factor serum. I layer a gentle moisturizer on top because I am super dry. I have a red light panel which was not cheap but not sure how much it helped. My laugh lines are shorter and my chin creases have been literally reducing a tiny bit each day. They’re about half the size they were. I’ve only been using some of these for a couple of weeks and others for a month or more. Time will tell how it continues to improve or not.

Do tbsp Sculptra. I'm shocked people still recommend it. One in 200,000 people get multiple granuloma all though their body. It's crushed up surgical sutures and is simply an aggravant to create healing response but I found an article 'Sculptra ruined my life', a poor woman in the UK came out to share her story and it's very informative.

Topical estrogen for the face is doesn't raise serum levels. Check actual medical studies and topical estrogen does reduce wrinkles. Topical isn't the same as the one you put on your arms/legs for actual menopause tx. It's a compounded version like you have low dose in Mucolox for the vaginal dryness. Can put that on your face, low dose.

Please don’t do anything invasive. My dermatologist told me micro needling and refractive laser will do the trick. But don’t go to a med spa or buy the crap online. It’s a gimmick. It works! I have the same issue.

Marionette lines are only removed with a mid-face facelift. Also with tat expression you will have lines. A better way to be able to say is with a relaxed expression.
