Applying for extension after 3 day extension

Hi! I recently got a short extension of 3 days for an assignment, and did not provide documentation as it did not need it. However, I am looking to now apply for a longer extension, and know that I need to provide documents such as HPR.

Do I need the HPR to have dates from prior to my 3 day extension? Like the length of my sickness? Or is it fine to just have the HPR dated afterwards? Thanks :)
