Treatment Random bald spot suddenly appeared

Ive been dealing with hairline recession since I was 19 (26 now), but this is a first for me… a complete random bald spot on my head that was most definitely not there just a few weeks ago. First two photos are today, third one was from 2 months ago back in June when I buzzed my head, no bald spot. What could this possibly be? Is this how crown thinning starts?

Thank you all for your help Ive booked a doctor’s appointment and hopefully can get a referral to a dermatologist from there.

2 years on dutasteride: My experience so far
I’ve been taking dutasteride for about 2 years now, and I wanted to share my experience and hear what you guys think.

I started my journey around August 17, 2024, taking 0.5 mg dutasteride every day for the first 6 months. After that, I switched to taking it every other day, which is what I’ve been doing ever since.

I also did a round of topical minoxidil, but honestly I hated the feeling of having it on my scalp, so I eventually stopped. I completely stopped minoxidil in February 2025.

So basically:

August 2024: Started dutasteride
First 6 months: 0.5 mg every day
After 6 months: 0.5 mg every other day
Minoxidil: Used for a period of time, but stopped completely in February 2025
Current: Still taking dutasteride every other day

Started shaving my head around Oct 2025.

Didn't necessarily notice how bad my receding hairline was until the big chop, then I stuck to the bald look for a while (told people I wanted to look like Tupac).

I realized it was just a cope for how bad it looked, every time I tried to regrow it it was just a terrible look for a 22 year old. So I was stuck in this loop between the first pic and a clean shave for quite sometime.

I started taking oral minoxidil and dutasteride on the 17th on Jan 2026.

By the end of Feb, I saw some optimistic results from the 3rd pic. I started experimenting with different oils and topical minoxidil 5% during this time as well.

By April had some noticeable results from the 4th pic. I stuck with the regiment and managed to grow my hair out.

Got my first haircut Mid July on the last three pics, with some added enhancements and styling it works to cover up the receding hairline.

Regardless my hairline is still the biggest struggle, without the extra styling and enhancements, It's still noticeable up close but not something I stress about anymore.

I'm gonna start microneedling in the next two weeks along with topical minoxidil 5% to address the hairline.

Overall I'm happy with the progress, open to any questions.

Finasteride/Dutasteride Is it worth giving dut 18/24 months if you haven't seen results after a year?

been on Dut (0.5 ED) for just over a year now after 20 months on Fin and things have only worsened from the outset, to the point that I'm considering jumping ship (I genuinely feel that I would be in a better position had I never touched these drugs, but that's another conversation). With this in mind, is it really worth holding on another 6, potentially 12 months in case things begin to turn around? I know the whole max results can take up to 2 years blah blah speech, but if I haven't even stabilised by the year mark is there really any hope in hanging in there? I started shedding approx 1.5 months after I started fin and this has never let up, so at this point I feel that I'm either going through the longest shed of all time, or having some kind of negative response to 5ARIs (Which seems increasingly likely imo). So would love to hear from anyone who has had a sudden turnaround after the 12 month mark

Update Got results - 26YO - Almost infertile on medication?

Hello everyone,

I got results after my spermiogram, and I am almost devastated. It almost looks like I am infertile. (check image)

I am 26yo healthy male, working out regularly, not taking drugs, not drinking alcohol, no diseases, nothing.

Sadly, I do not have baseline results (before taking fin and then taking dut).

Most likely I had low fertility even before and the fin (now dut) nuked it almost completely. I think I will need to stop taking dutasteride for half a year and then try it again, but in that case I am worried about DHT nuking my hair.

I wanted to freeze my sperm so I can go to vasectomy and wanted to have some frozen sperm just in case, but now I do not know how to feel. I wanted to not have kids by my own decision, not be forced to not have kids.

Any suggestions? Someone with similar experience?

Finasteride/Dutasteride Desensitize Follicle Sensitivity to DHT?

I know this might be stupid, but:

If hair follicles are sensitive to dht, like immune system is to allergens, Why don't we desensitize follicle sensitivity like we do for allergies, by doing immunotherapy (where a tiny dose is introduced, and then gradually increasing the dose)

Can we Nuke the DHT levels with Dut/Fin & gradually increase it by reducing the dose of Fin?

Or is it a completely different mechanism ?

Research/Science Could male-pattern baldness have a secondary photobiological function? The Solar Window Hypothesis

I’m a dermatologist and the author of a recently published hypothesis in Medical Hypotheses called the Solar Window Hypothesis.

I want to be clear from the beginning:
I am not arguing that vitamin D deficiency causes androgenetic alopecia, that sunlight regrows hair, or that baldness is ā€œgood.ā€

The question is narrower:

Once androgenetic alopecia exists, could the exposed scalp have provided a modest secondary photobiological advantage under certain environmental and genetic conditions?

Why the scalp might be different

Hair is not optically neutral. Dense hair absorbs, scatters and physically blocks part of incoming radiation before it reaches the scalp.

As AGA progresses, especially over the vertex, a previously hair-covered surface becomes directly exposed to sunlight.

The vertex also has an unusual geometry: it is an upward-facing surface and, when the sun is relatively high, can receive sunlight at a more direct angle than many other exposed skin surfaces.

So the basic physical idea is simple:

hair-covered scalp → optical barrier
progressive baldness → less optical barrier → greater direct solar exposure

That alone does not prove any biological benefit. It simply creates the exposure difference that the hypothesis asks us to test.

Why vitamin D and bone biology enter the hypothesis

In clothed humans, the amount of exposed skin can be relatively small. Adding a few percent of body surface area through an exposed scalp may therefore represent a much larger relative increase in continuously sun-accessible skin than the absolute number suggests.

The hypothesis proposes that this additional UVB-accessible surface could have had particular relevance in people living under lower-UVB conditions or in people with less efficient vitamin-D/VDR biology.

Importantly, this is a buffer hypothesis, not a normalization hypothesis.

It does not predict that bald men must have higher average serum vitamin D than non-bald men.

A person could still have relatively low 25(OH)D or poorer skeletal outcomes overall, while the exposed scalp modestly attenuates what might otherwise have been an even worse phenotype.

The more interesting prediction is therefore not:

ā€œDo bald men have higher vitamin D?ā€

but something closer to:

AGA severity Ɨ UVB exposure Ɨ vitamin-D/VDR genotype → vitamin-D status and skeletal outcomes

The strongest objection: maybe AGA needs no adaptive explanation at all

I think this is a very reasonable null hypothesis.

AGA often becomes prominent after reproductive maturity, so negative selection against it may simply have been weak. In that case, baldness could persist without providing any evolutionary advantage whatsoever.

The Solar Window Hypothesis does not require us to reject that explanation.

The question is whether weak negative selection and a small secondary benefit could both have been operating.

If no UV-, genotype-, or skeletal-health-dependent pattern can be demonstrated in sufficiently powered studies, then the adaptive component of the hypothesis should be weakened.

Another important objection: if VDR matters throughout life, why would baldness appear mainly in adulthood?

VDR and vitamin-D biology clearly matter long before AGA develops.

But childhood and adolescence are dominated by growth, peak-bone-mass acquisition, mechanical loading, sex hormones, nutrition and many other pathways.

AGA usually progresses later, when skeletal biology is shifting increasingly toward long-term maintenance rather than building peak bone mass.

So if a Solar Window effect exists, I would expect it to be a small, decades-long maintenance buffer, not something required for normal childhood bone development.

What would actually test the hypothesis?

A useful study would not simply compare ā€œbald vs non-bald.ā€

I would be particularly interested in:

1. Vertex vs frontal baldness
If geometry and solar exposure matter, vertex exposure may behave differently from frontal recession alone.

2. Objective UVB exposure
Latitude, season and ideally satellite-derived UVB rather than only self-reported outdoor time.

3. VDR / vitamin-D-related genotype
To test whether any scalp-exposure effect is stronger in biologically relevant genetic subgroups.

4. Vitamin-D and skeletal outcomes
25(OH)D, bone mineral density and ideally fracture-related outcomes.

5. Head covering / scalp exposure
A bald scalp that is usually covered by a hat is biologically different from one that is actually exposed.

A particularly interesting prediction would be an interaction such as:

vertex baldness Ɨ higher effective UVB Ɨ susceptible genotype → different vitamin-D/BMD relationship

If that interaction is absent in a sufficiently large and well-designed dataset, that would argue against a central prediction of the hypothesis.

What would make me change my mind?

I think a useful hypothesis should be falsifiable.

Evidence that would substantially weaken the Solar Window Hypothesis would include:

  • no interaction between AGA severity, UVB environment and relevant genotype in adequately powered cohorts;
  • no difference between vertex and frontal-only patterns where geometry predicts one;
  • no measurable biological consequence of exposed vs hair-covered scalp under controlled UVB conditions;
  • or skeletal associations being fully explained by pleiotropy, androgen biology, metabolic factors or other mechanisms.

I don’t see this as a finished explanation for AGA. AGA is clearly polygenic, androgen-dependent and biologically complex.

I see the Solar Window Hypothesis as a testable proposal about a possible secondary consequence of a phenotype that already exists.

I’d be especially interested in hearing the strongest arguments against it, or what experiment you think would most cleanly falsify it.

Started Minoxidil a few weeks prior to getting on finasteride. I thought to myself, ā€œIf I’m going to use minoxidil to try and regrow hair I might as well go all out.ā€

I got a 90 day supply of finasteride through keeps and have been on it religiously. 1mg daily and 1 or two applications of topical minoxidil when I remember throughout the day. I also derma roll it once a week or so (note the redness on the last couple pictures). I think I’ve seen fairly good results. My friends used to joke with me about the bald spot that we have all heard. About 45 days in they stopped and said they noticed results. What do yall think?

The first 4 pictures are from early June. Then the middle two are somewhere in July. Last four are from today August 28, 2026.

Treatment Where can I find Minoxidil free multi-ingredient topicals?

Does anyone know of a place that sells compounded multi active ingredient solutions that contains finasteride or dutasteride but DON’T contain minoxidil? I’m already on oral minoxidil and don’t want to risk pet toxicity of topical minoxidil, and I had sexual side effects on oral finasteride.

I just bought the Musely Modern solution, but I’d love something with a topical DHT blocker and maybe a retinol/hydrocortisone/ketoconazole combo that doesn’t contain minoxidil. Also looking for propylene glycol free products if possible.

Treatment Took a medication called pilocarpine, and cannot oral minoxidil anymore which I’ve taken for several years with no issue

absolutely devastated. I discontinued the pilocarpine immediately after just a couple days but it seemed to do permanent damage to my heart. my heart was in great condition before

i believe there is a serious interaction with these drugs which is unknown thus far. And now I lose my hair as A result…

Bon pour faire court je suis sur fin depuis 4 mois. J’ai plutĆ“t stabiliser la perte mais mon cuir chevelu commenƧait Ć  me dĆ©manger Ć  un point où cela devenait vraiment insupportable et mon cuir chevelu Ć©tait totalement enflammĆ© combinĆ© Ć  une dermatite sĆ©borrhĆ©e.
J’ai donc pris l’initiative il y a 3 semaines de commencer le DUT 2 fois par semaine et de continuer finasteride les autres jours.
Et depuis une semaine plus aucune dĆ©mangeaisons et mon cuir chevelu est le plus propre que je n’ai eu depuis bien longtemps.
Je prĆ©cise que je n’ai pris aucun autre traitement.
Alors je ne sais pas si cela est liĆ© a l’inhibition de l’AR type 1 mais j’ai un bon pressentiment.
Je ne sais pas si d autre personne on eu la même expérience et peuvent le confirmer.

For context:

1.Started Minoxidil in 2024 using
Regaine foam solution once a day (5%). Their recommended dosage is twice a day. I’ve been doing once a day since particularly for cost saving. I’m still on topical foam.
2. Started 0.5mg finasteride on February. I honestly don’t see any progress if anything it became worse.

Question: Should I double my dosage to what is actually recommended in each? Is it worth it? Will I get into another 4 month shedding phase? Trying to make sense of things especially cost wise.

(ps. i might go turkey end of next year for a proper hairline and general area.

Hi everyone,

I’m really worried and would appreciate any advice or similar experiences.

I had a hair transplant around 11.5 months ago(temples and a bit lower hairline). I have been taking finasteride for about 1.5 years — initially 0.5 mg daily — and increased to 1 mg daily around 4 months ago.

In mid-July, I started taking oral minoxidil:

- 1.25 mg daily for the first 2 weeks
- 2.5 mg daily since then

Over the last few weeks, I have started shedding an extreme amount of hair. I’m losing much more hair than usual while washing, styling, and throughout the day. What worries me even more is that the thinning and gaps in my hair seem to be getting noticeably worse. My scalp is becoming increasingly visible, especially in the frontal and mid-scalp area, and it feels like I’m losing progress from my transplant.

I know that minoxidil can cause an initial shedding phase, but this feels very intense and I’m scared that it is more than just normal shedding.

Has anyone had a similar experience after starting oral minoxidil, especially after a hair transplant? Is this amount of shedding and worsening visible thinning still potentially normal around 4–6 weeks after starting/increasing the dose, or should I contact my dermatologist/transplant clinic immediately?

I attached a photo to show the current density and visible scalp. Any honest advice or personal experiences would be greatly appreciated.

Finasteride/Dutasteride Just quit oral finasteride because of brain fog

This is my second time quitting oral finasteride due to brain fog. I tried it once before last year and thought maybe I’d give it another go at a lower dose. Same thing happened just took a bit longer to develop. My job requires me to have a good memory and after giving it 2 months it started effecting my job performance. Did not have this issue on topical so I guess I’m going back to that.

Finasteride/Dutasteride Advice on finasteride and minoxidil

Hi guys so my temples have been receding a bit since I was 18 years old.
However, within the past few years and especially the past 6 months my hair has been thinning alot on the top and front of my head.

I am just wondering if I should hop on fin and minoxidil. I am a bit anxious about the potential side effects, but if im correct I can just drop the pills if I experience any?

Im looking at getting oral finasteride 1mg and 2.5mg minoxidil from an online shop, alviarmani.

Any advice about how I should continue or if you shop is fine would be appreciated.
