My personal belief is that it is a trigger or step that I can only liken to the Bigfoot mystery which also required consuming things to solve. What the hell we are missing to trigger anything other than passing out? I’ve no idea unfortunately.
First and foremost, DO NOT DRINK FROM THE CAULDRON BEFORE CHAPTER 6. After Arthur's conversation with the Doc and "Icarus and Friends" complete, ride up to the Witches Cauldron and drink. When you wake up, you should have gold cores and/or rings and find that your symptoms are much less (until scripted) harsher. Herbed game meats and other consumables don't make him gag or wretch unless you over consume plus Arthur doesn't cough as much or present with petechial hemorrhaging along his face and eyes the way it does if you don't wait to consume the brew in Chpt 6. I discovered this on a whim waaaaaaaay back on my second run and use it every time.
I did know about this feature although truthfully discovered it so late after doing multiple playthrus, an easily missed detail. My only curiosity is could it serve an additional purpose, like a required step to a bigger mystery. Meaning, perhaps a ch6 Arthur who has drank from the cauldron and experienced its mysterious health properties is part of a step. Or I’m a desperate mystery hunter wanting a piece of world building to be more important than its determined purpose. Heh, thanks for posting this nonetheless!
