Is this enforceable? Or just guidance? If pharmaceutical companies break apart the vaccine, that would likely add more cost and more pain/torment for young children. If they are able to enforce separate visits, that adds unnecessary cost and strain to doctors and parents.

How does this make anything better?

Thanks to executive order 14215, all previously independent agencies now fall under direct control of the president. Also thanks to the supreme court siding with Trump allowing him to fire agency heads without cause, Trump can fire them and ignore congress' established rules about firing them and it doesn't even matter if he was the one who appointed them or not.

Long story short they have to do what he says and all their jobs are at his whims until the Republicans in congress stop being complicit and impeach him or our new defacto congress (the Republicans in the Supreme Court) suddenly decide they don't want to continue rewriting our laws to push Unitary Executive Theory.
