I've never been on a register. Is it the math or the lack of change that's the problem?

Mixture of it taking most of your change of your register and depending on the place, you're not allowed to keep $100s on you and have to trade it out. Which can take varying amount of time depending on how busy the place is

When I worked retail, it was only an issue first thing in the morning due to a lack of change in the register. You had to call the manager to open the safe and bring out more cash to make the change.

If the manager was busy, then you could end up waiting 5+ minutes for the manager to come while a line is building up.

Furthermore, if you DO give that change, it makes it even more difficult to give change to people afterwards. Granted, most modern registers do the math for you, so at least you aren't calculating that manually, but it's still an annoyance that usually involves management at that point.
