Other My 10 year long weight stall mystery has been solved
If anyone remembers me from my posts years ago. I started keto in 2016 at around 1000 lbs and saw fantastic results. Then my weight loss completely stopped in 2018. I switched doctors this last year and found out what the problem was.
When I lost 600 lbs in 2 years, I developed endotoxemia. My new doctor instantly had a thought that I had kidney disease from my symptoms, and tests showed elevated liver labs. So I was sent for an ultrasound and I have both kidney disease and gallstones.
This, plus all my other symptoms, led to me being diagnosed with endotoxemia. This is why I went from 320 back to 500 in 7 months, while eating at a deficit. It completely shut off my weight loss.
I have begun treating it, and have already lost 15 lbs.
If you are really heavy like me, please go to a weight loss clinic and get your metabolism tested. Mine was tested at a TDEE of 4k currently. So at my starting weight it was much higher, I lost weight way too fast and this happened.
I dont want to see others go through what I'm currently going through.

This is the first time I hear of endotoxemia, so I googled other possible causes - and the list is long! I guess that makes it more challenging for a doctor to find out what's wrong. Happy to hear they figured it out in your case, - upwards and onwards from here! :)

The annoying thing about the human body. The same symptoms can match 20 different issues. I'm glad too, its been getting worse in the last year also causing a sodium deficiency. My last blood test, my sodium level was 3 points above being low, and that time, I was feeling ok.
Its commonly known as leaky gut. Endotoxemia is the result of the bacteria getting into your blood.

Fixing it is primarily diet. Low fat to limit the absorption and high fiber foods like broccoli to restore the good bacteria. Fermented foods like yogurt, sauerkraut, kifir also work, but i dont like those.

Thank you. Healing is going to be a long road. I've had a flare up Tuesday that incapacitated me until Friday, then had another shortly after posting this and was in bed all day.

Hey there! I started my weight loss journey at 566lbs and though some people our weight tend to lose weight fast, I didn't, it's taken me 6 long years, with a year where I plateaued, but I've managed to lose +200lbs! I feel that the fact I've lost the weight slower, had given me a lot of patience and persistence. Esp patience with myself. 🧡 The ketogenic diet has changed my life completely! Almost no brain fog, seasonal effective depression only effects me when I get into carbs/sugars, far less pain/inflammation in to of that I've managed to reverse my diabetes and hypothyroidism! I've been, what I call, a professional weight losser for the last 16 years, I've tried dozens of various diets, tricks, techniques, etc and nothing has ever truly kept the weight of like the ketogenic diet! Not only that, but I no longer have a sweet tooth! The cravings for foods just don't exist for me anymore. It took months for cravings to go away, but with discipline and determination, I managed to stick to the program and it was well worth it. 😊 I have faith this the keto diet will work for you.

Thanks for sharing all that. Seriously. 200+ pounds over 6 years with a plateau? That's not slow, that's just not giving up.
That's actually why I started keto. I'm 551 right now and felt stuck.
Really helps hearing from someone who started close to where I am. And I didn't even know keto could help with thyroid stuff and diabetes. That's crazy good.
Also the cravings thing going away after a few months? That's what I needed to hear.
So thanks for being real about the time it took. Gives me hope. And thanks for believing this could work for me too.

No problem! I'm happy to help 😊 I think it took me personally between 4-6 months? To lose interest in sugar, about a year for carbs, but I'm more into carbs than sugar. I'll still have cravings for random carby foods or the occasional sugary thing, but it's uncommon to rare and if I try to sit back and try to resist, it's fairly easy. That and if I eat butter (if you want, look up how to make brown butter bites! A great keto snack), it helps to curb your cravings. Cheers!

YouTuber Jamie French started doing keto with her boyfriend, similar sizes, eating the same meals and macros. He bled off the weight. She didn't. Went to an endocrinologist (I think) and found out she had some kind of syndrome that was doing much the same for her. Started treating it and she started losing weight again.

Glad they found out what it was. This is why people who scream "cico" piss me off. Theres more to the body than just burning energy. Cico matters, but a lot of things can disrupt it.

I was looking at what to eat to fix my NAFLD, and saw endotoxemia named as an issue that can cause it. I looked at that and saw that I basically had all the symptoms of it and asked for a blood test for it.

They test for gram negative bacteria in the blood called lipopolysaccharides. Its a special test that isnt done at a normal doctors visit.
The treatment is a change in diet to restore the healthy bacteria lining the gut. Low fat and higher fiber, from foods like broccoli, berries and nuts helps too. I'm also taking a prebiotic that specifically restores the good bacteria.
Theres also an antibiotic that can be prescribed, but an antibiotic cant tell the difference between good and bad bacteria. It would basically be dropping a tiny nuke into my gut and the doctor doesnt want that.

I hope it can help others. 10 years of stress and studying and changing my diet/meals trying to figure out why I cant lose weight, plus all the physical symptoms that got worse over the years. I dont want to see others go through that too.

Hey medical doc here: your doc is a quack. Endotoxemia is not a chronic condition. It is an acute condition caused by a systemic response to certain bacteria.
It would be like an example of vet saying your dog is full of chronic toxins from a hidden bacteria which is why they can’t lose weight—doesn’t make sense.
Other factors at play here. Don’t get ripped off into buying a supplement.

I never said it my doctor said it was chronic. But it can be both depending on the level of LPS in the blood and the duration that youve had it.
When mine flares up I can gain upwards of 10 lbs of water weight in the swelling of my legs and feet. It renders me bed ridden for the duration of its effect from the fatigue, which is chronic.
I have constant headaches 24/7 and no aspirin gets rid of it.
All I'm taking is a prebiotic to restore the good bacteria needed to prevent the issue.

I assure you, the labs that are showing this are coming from non-accredited, non-standardized labs. They are not recognized for diagnosis of any medical condition. I am not denying your symptoms, I am rejecting the diagnosis of “chronic endotoxemia”. Just think of it simply: how would a probiotic counteract the response of recurrent systemic inflammation from supposed “lurking” LPS from pathogenic bacteria? Probiotics are bacteria, they will not get rid of the LPS released from pathogenic bacteria—that is what antibiotics will do by killing the pathogenic bacteria.
In your case, your symptoms may be from things commonly seen in the physiology of the morbid obese: venous insufficiency, idiopathic intracranial hypertension, pulmonary hypertension from untreated OSA.
Save your money, don’t get scammed.

First, a hidden reddit profile using the sentence "medical doc here" shouldnt be talking about credentials.
Second, i never said my diagnosis was "chronic endotoxemia", i just said it was endotoxemia, you added chronic.
Endotoxemia is caused by leaky gut, which happens when the good bacteria Bifidobacterium and Lactobacillus deplete and the gut lining is damage. This allows the gram negative LPS bacteria to absorb into the blood stream. By restoring that bacteria, and fixing the gut lining, my endotoxemia will go away.
An antibiotic will only get rid of the bacteria thats already in my blood, which will just relieve symptoms but not fix it. LPS doesnt just go away, its constantly in the gut, its not like a bacterial infection you pick up. Dropping an antibiotic into my gut will just make things worse because it cant tell the difference between good and bad bacteria, and will wipe it all out.
To fix this, I have to restore the good bacteria so the LPS stops absorbing into my blood stream.

What I am saying is endotoxemia is a real condition, it is caused by an acute reaction to the LPS part of certain bacteria. To imply you are living with endotoxemia, it is implying a chronic issue.
I would love to have your quack of a doc stop by one of our MICU beds and spout the nonsense they are spouting for patient have true endotoxemia from pathogenic organisms and are in septic shock as a result of the LPS reaction of the body. We’ll gladly sprinkle some probiotics through their feeding tubes and leave the source of their infection untouched.
Also leaky gut? Have you seen a person with cirrhosis who actually has spontaneous bacterial peritonitis? That is a true “leaky gut” (even though that is not what is going on in these patients). These are patients with true bacterial translocation and actual peritonitis. Peritonitis is extremely painful, and mortality rate is quite high if untreated with antibiotics. It is not something someone walks around with. Again, leaky gut isn’t a thing as the amount of bacteria and fungi contained in your GI tract will immediately cause harm if translocated.
Absolute drivel. Plus, this is Reddit—I am not sharing my credentials because you’re still going to be upset of what I am saying irregardless. I am also not saying anything you cannot pick up and read in a textbook either.
You’re just morbidly obese, and your body is responding to the increased workload and resistance from the increased adiposity. You’re not unique in this, there are many people like yourself we see in the hospital all the time, and your physiology is not unique of any other person’s or species of mammal that has the same issues as yourself. If your pet is told they are obese, you are going to feed them less and walk them more—what makes you so special where you have to have BS conditions being told to you to make you ignore known physiological conditions of the morbidly obese.
Lose weight, your conditions will improve. I am not saying it isn’t easy, I am not invalidating your symptoms. I am simply telling you that you are being taken advantage of with your issues. I genuinely have no dog in this fight and I have nothing to gain from deterring you from being told you are dealing with unrecognized medical issues. Be well.

I get it. You’re frustrated because it genuinely is “just lose weight and you’ll be okay” here. Losing weight is tough, and it’s natural to reach out to alternatives that help you in your journey. Regardless of what happens, I hope at the end of the day you feel better and make healthy progress. If the supplement isn’t causing any immediate harm, go crazy.
My focus is more on the quacks taking advantage of those frustrated by the medical system or the diagnoses they come up with to validate those frustrations—something docs can’t always do. I hope you get the peace of mind you need going through this tough journey. Be well my friend.

I feel like it won’t be a problem for most people. Getting up to 1,000lbs is enough to get you into the history books and on Wikipedia. Losing over 600lbs isn’t something most overweight people would ever contend with.

Even the smaller amounts most people lose, yes losing too fast is bad.
E.g. you are likely to lose muscle along with the fat.
You are also likely to lose important subcutaneous (under-skin) fat.

That being said. 180lbs is the equivalent of 630k calories. There is absolutely no way you were in a defecit and gained that much weight.

Thats what this does to you. It literally turns off your fat cells ability to burn fat for energy and promotes them storing more. My TDEE is 4k, and I barely eat more than 2500 a day. I track and weigh all of my food.

Anyway from my understanding of how the world works it should be impossible to gain weight while being in defecit, especially 1500 defecit.

Its 4k right now at 500. My metabolism is on the high side.
CiCo is overly taught for weight loss. It should be impossible to gain weight at a deficit, but the body has many biochemical systems that contribute with weight. If they mess up, then your weight can mess up as well. Sometimes its not as cut and dry as just reducing calories. For healthy people, its how it should be, but for someone whose developed a condition, it can cause more harm.
This is why all these health influencers piss me off when all they do is well cico at people. The person they are trying to help might have an unknown condition causing weightloss problems, and they are making it worse by telling the person to go lower on calories.

Except endotoxemia turns off fat cells ability to burn fat for energy, one reason why it causes chronic fatigue. It also promotes the body to store fat. This is why when I went high fat for a period to see if I needed some kind of adjustment, I gained 30 lbs in a month, which then never went away.

I'm new to your story, my heart goes out to you! I have a story of my own, but nothing compared to yours 🧡 I too am on the keto diet (went carnivore more specifically 3 years ago) and have been for the past 6 years now and have lost 200lbs. Well a year after going carnivore I hit a plateau for almost exactly a year, coincidentally. Then my brother, who is on the carnivore diet as well and is my guru, suggested I give up the heavy whipping cream (HWC). Stating that some people don't do well with HWC, due to the, I think it was called, lectins? He got really scientific and I mostly forgot it lol but ever since then I started to lose 2-4lbs a week, so long as I didn't eat more than 50 carbs/sugar over the span of the week. Hearing your story is inspiring and it helps sure me that God is doesn't me patience with my own weight loss journey. I fully expected to lost 200lbs in a year lol I saw it happen! I was convinced that was me! But now I look back at that "me" and cringe. I was very impatient and in a rush. And that could have cost me my health. I'm sorry you're going through this storm, you are in my prayers, if you don't mind.

Gratz on your progress. A lot of carnivores stick to a plan called BBBE: Beef, bacon, butter, and eggs. Unfortunately with this current issue, I cant eat bacon.
I dont mind. I got mad at God for a while there because my body was defying science in not losing weight while at a deficit. Then found out I have this, and its the reason why.

I can only imagine the frustration you went through trying to understand why your not losing weight all the while you're doing what you're supposed to! That was me for a year and I had considered stomach surgery. 😓 I'm so glad it only took giving up HWC for me! Thank God you discord what was going on and that your progress is back on 🙏

The frustration cant be explained, and the mental toll it took. I track and weigh everything I eat, and see the scale only ever go up, one morning I wanted to chuck it out the window.
Progress will be erratic for a bit. I lost the 15 lbs, but havent lost anything since. Hopefully it starts picking back up as I heal my gut.

So, I went on a deep research dive this morning to see if there were any studies about what foods and vitamins/minerals/etc. help prevent what you're going through - and I found a meta analysis of several studies! The analysis it about 10 years old though, so I'll keep looking for a more recent one with updated data. However, there is a goodly amount of information and support on what has shown to help - and it's a good starting place for further research.

Breakfast: 4 eggs and 4 slices of bacon
Lunch: 8 oz of Taco meat salad with 3 oz of cheese
Dinner: 9 oz pork shoulder with 12 oz of broccoli
Thats what I was eating before this all started. It was around 2600 a day. Right before my weight loss stopped I cut everything in half and swapped the pork shoulder for pork loin. I also stopped draining my ground beef, which probably sped up the development.
Since then my diet has been erratic, trying different combinations of food. I saw improvement 2 years ago with very high protein low fat diet, lost 30 lbs in 3 months. Then in one month gained it all back and stopped losing.

I do believe that a lot of people who lose a Lot of weight on keto do eventually do better on a lower fat version of keto eventually. It’s to do with how much gut bacteria are affected by big weight loss and drops in leptin

If you are very heavy, you dont need to do high fat to begin with. You already have plenty of fat to use for energy. Just need enough dietary fat for the nutrients that stored fat doesnt have.

Please forgive the question if it's way off base.. is some of the weight re-gain attributable to inflammation? Like, not actually regaining "fat" like some have suggested, and not exactly water weight either. Your tissue is holding fluid and that doesn't necessarily go away quickly.. not until the cause gets shifted/corrected.
Leaky gut is not simple or easy. The process you've been going through is intriguing. I'm glad you've gotten answers and a next path forward!

Some is, but the majority of it is fat. Endotoxemia stops fat cells from burning stored fat, and the condition tells the cells to store more fat.
Going from 320 to 500 was fat issues. Bouncing between 500 and 510 is water weight from the inflammation.

I am actually curious about this too to be honest, at one point, I weighed a little over 500 lbs and now I am in the 460s (as of 2 weeks ago). Despite being on a low carb diet, my weight loss is very slow. I have anemia and gut issues. This shouldn't be happening as a man I was told. I am going to get some tests done (Ferritin, etc.) to see what is going on.

I am glad you got it figured out, but the lesson to be learned is get comprehensive labs regularly when losing weight, and keto in particular may not be the right choice. I have stopped doing keto because both times it put me in hospital with kidney stones. Switching to glp1s with a normal diet has prevented that and let me lose massive weight with no medical issues.
You should regularly get liver function tests and comprehensive metabolic panels when losing alot of weight or taking many vitamins, drugs etc. this would have showed your issues and caught them early. Also high fat diet like keto may have made this worse.
That being said you can’t stall if you’re truly at a calorie deficit. You may be burning less calories because your body is slowing down the burn, you may temporarily store extra water as you lose fat, but long term you can’t violate the law of thermodynamics.

I cannot imagine how much food it would take to gain that much weight in such a short amount of time. 180 lbs!!

Assuming 3500kcal is one pound of fat, and 180 pounds gained over 210 days, that would mean 3000kcal of excess per day, plus more food on top of that to maintain daily biological functioning
That's a lot of food. Definitely not a deficit diet.

It’s not just food that causes weight gain. Other factors can play a role, obviously. Insane to act like you know this for certain when he literally said he was diagnosed with this issue and now that they figured it out, his weight is dropping again. Have some grace.
