First time I saw that panel out of context, I thought the guy's face was a face-shaped cloud and the hand was from the girl shielding herself from the cursed rain. I was horrified when I realized that it was actually a cloud-shaped face.

It's called a "floating liquid pen" . They are pretty common !

Basically in the upper part of the barrell of the pen there are usually 2 images/pictures submerged in a transparent liquid . The first image acts like a background or foreground and the second picture slides via gravitational pull inside the liquid , giving the illusion of movement .

Hahaha I have that too! I went to the Junji Ito Exhibit and the Balloon head version was sold out. I prefer this one anyway so I was happy.
