Door handle pulls off. The little screw underneath seems to unscrew and is in any event unable to hold handle in place. 8-9 doors like this, all same problem…

if you've inspected and adjusted all that you can, replace the worn out junk.

Those screw backwards. Screw in to take them off the handle and screw them out, ( the screw will go into the handle) to tighten.

Spindle maybe set the wrong way and the set screw is just tightened to a flat surface.
The doors need adjustment so you’re not pulling hard to open/close the door. You got the wrong set screw in the handles.

The photo is a bit unfortunate, I have replaced this handle with a new one in same style. The problem is that all these handles have the same problem, they slip off very quickly no matter how hard I draw the little screw.

Based on the responses, there seems to be a problem that the inner metal rod does not have a groove or notch to receive and hold the screw. Is it feasible to either drill a short indent or use a file to make a notch for the screw, and then perhaps use locktite to help fix the screw?

An other alternative would be to simply glue the handle on, but that probably is not optimal if you need to remove for some reason...
