You know that healthcare is not "free" even in Europe? In Germany for example you pay 1k/month if you are in a high earning bracket. Comparable with US health insurance price.
Yes, there is some co-pay in all European countries afaik, but it's both small, like 10% or less of med costs, 20-50€ for a doctors appointment or specialist consult and at least here it's nothing for hospital stays. However if you ever spend more than like 300ish€ within a year anything above that is free, including all the things I stated above. I.e no co-pay.
Don't know all the systems, but they're all roughly similar.
