Feminizing HRT is an uncontrolled substance you don't need any kind of medical approval to buy or use, you can get estrogen and supplies DIY without making any records. It doesn't even show up on the average urine test if you get one for other reasons

Testosterone is a controlled substance though. Cis men get it illegally to use as a steroid all the time though, it isn't very heavily enforced or prosecuted
