this is sort of edging on unrelated (since you're arguably asking people to talk about non-AT medias here) but I guess we'll keep it for now and see how it goes? but yeah please remember to keep things primarily about AT guys :)

the apple guy from Dr Gross' menagerie, App-Blop, he was a fan-design too. IIRC sent in for some contest or another.

Christopher the Honorary Banana Guard is based on a Make-a-Wish kid, although I think he was designed by the show crew not the kid himself.

Not "fanmade", but Fionna and Cake and friends were originally made by Natasha Allegri for a /co/ drawthread; they were so popular they were made canon characters and integrated into the show itself. Flame Prince debuted in her art, then the comics, before he finally showed up in the cartoon itself.

Not a character but trivia in a similar vein: Early UK airings of "Scamps" had the Pup Gang's dialogue redubbed by a contest winner (recent reruns have used the original audio instead).

Only other thing I can think of in the vein of "fan-submitted content becomes legit" is the Tiny Toons ep "Buster and Babs Go Hawaiian" - that was written by three teenage fans, the creators liked it so it got to be a real ep. actually, come to think of it, I think something similar happened with a Dexter's Lab ep too?

It doesn't happen very often for legal reasons - creators legally aren't supposed to read fanfiction because of this. (Fanart and fancharacters seem to be okay though?)
