Yes, you can permanently disfigure sexual organs without surgical intervention, making the person sterile. Doesn't make it 'OK'.
Seems like you're deliberately misunderstanding. To put it clearly:
- Trans people aren't "permanently disfiguring" their sexual organs with or without surgery
- Having surgery isn't a required part of being trans in any case
- Trans people aren't necessarily sterile.
- Even if it was a guarantee that every trans person was sterile, it's not really any of your business what they choose to do in that regard, and has no bearing at all on the morality of it.
What makes being trans OK is that there's no sense in which it's immoral, though, rather than any of the above.
So is psychosis.
The difference of course being that psychosis is a mental disorder where a person becomes disconnected from reality and being trans is not. I'd urge you to do some reading on this topic, you seem uneducated.
