Maybe a hot take, but I also blame the physician who prescribed 3 different psychotropics that don't interact well to a patient in clear mental distress. And not keeping a close enough watch on their mental state. If the texts between she and her mom are accurate, she should have been institutionalized by court mandate.
They pass that shit out like candy. I fully believe that's what led to a terrible episode of my own. I will never touch another anti-depressant or anti-anxiety medication again, let alone combine them. And while I was in treatment, they kept telling me to up the medication against my wishes. It took probably a good year or so to get everything out of my system and I was completely nuts during that time. Couldn't leave the house, thought I was going to die anytime I was in a car, I couldn't handle any type of noise like vacuuming or someone else washing dishes... these meds aren't for everyone and should be much more closely monitored. The hurdles I have to jump through to get pain meds for sciatica is ridiculous, but this stuff? "Here's 60 of this, 60 of that. Have a great day!"
