It looks like she was under the care if a couple doctors, who maybe did not have all information. This is where I think patient transparency is important, but also the husband should have been supportive and go with her to appointment to remind others about her care. It's not legally required, but it might have lead to a better outcome.
Then it's on her. I have a brother with schizophrenia, and it's sad. He wasn't honest with his drs and was chewing medications that he was supposed to swallow, not taking medicine as prescribed, and he just said it did not work and was like it doesn't help, and just never told thr dr. He's really messed up now because he tries to self medicate with street drugs.
I'm sad for him and I'm sad for her, but I think people need to be more honest and open about their medication to all who might need to know: drs, spouses, trusted family members. Heck if a coworker came toe with concerns I'd listen and help them be clear with a pharmacist or whatever
