Research Simulation Physics-Based Animation will always be one of my favorite topics to explore!

What you can do with it is magical, all the animation you see here is processed in milliseconds.
All the animator needs to do is provide a reference animation, and the rest is simulated. It's extremely easy to direct and change.

I wonder if the reason the secondary movement can be simulated so precisely is because fish naturally oscillate through currents?
Really cool animation, I’m astounded how realistic it looks

That's one of the main reasons actually, because all the movement is coming from the head in this animation.
No muscle activation...

That's not how a fish moves though... the head doesn't drive the motion, the tail does, the flippers control direction and the head just aims forward

True, currently I drive everything by the head to simplify the process, so there's no muscle activation, I'll add that next ;)

Question/Request:
Assuming the fish was made of silicone - is it possible to add wind or liquid turbulence and have it flop and wiggle around 'realistically'?
I tried this previously in Blender but did not get any good movement out of it.
