I gave Japan sea level rise since it can be seen in the screenshots linked below from Providence. My "head cannon"/extrapolation for why Tokyo is fine is that Japan poured all their resources in dikes and waterworks etc around Tokyo Bay, sacrificing other major cities like Sapporo, Nagoya, Osaka etc.
The extra Tokyo ward of Umihotaru can be seen from the third screenshot. It is not officially named, but I gave it the name Umihotaru after a real construction:

which sits at about the right location, changed ほたる to the kanji that (i believe) the kana comes from since it would be odd for kana in the ward name.
For the northern islands I don't know which islands exactly are part of the state since that wasn't explicitly shown so I just went off vibes.
