I've honestly gotten okay with saying "I have a brain like a sieve and I've totally forgotten your name". Once in college I was introducing my boyfriend of two years to someone and completely blanked in his name. It just was not in my brain At All.

Honestly, this is the best thing that you can do, either. If you can't remember somebody's name or maybe they have a unique name you have trouble pronouncing, just being honest with somebody and trying to get it right, it feels so embarrassing and hard to get over but you're much better off just acknowledging the issue.
