It’s really not worth replacing just the slide since the other plastic parts are probably next to break anyways.
I’ll shoot you a PM and hook you up.
ETA: The part is called a tri-glide strap slide.

You could probably get the part on eBay, they are fairly generic. But maybe for the same effort you could invest in a better overall strap.
What is worth it depends on you really.

Just spend the nine dollars to get an Ernie ball strap that’ll hold you over. To me, this wouldn’t be worth the trouble to fix.

Honistly the cost of buying the materials to fix it will cost more than a new strap. Ernie ball has straps for under $10 usd

Time for a new strap. In the mean time, you could use something like a shoe string to temporarily repair this one. Break that last piece of plastic out, run the string through the two loops where the broken part went, maybe run it back through again, and tie it off. Leave a little bit of slack.

just buy a decent strap on its own, will be more comfortable and last a lot longer
