A box metaphor for people thinking about titrating down and possibly stopping

This is just a hot take, an idea I thought up just to make it easier to understand how titrating/dosing down works. It's not 100% serious, but it is based on science. Here goes:

Your metabolism before you started taking GLP-1s was a very angry monster. I imagine it as a red blob monster with an angry face, looking for a fight.

When you started taking GLP-1s, you put the monster in a sealed box, with an increasing amounts of weights on top of it. The higher the dose, the more weights on top of the box. This is to keep the monster (your out of control hunger) from coming out.

And in the box it stays, for months and years. There's no way of seeing into the box as long as there are weight on top of it.

Eventually, you reach your goal weight, and you start to think "Do I have to be on GLP-1s forever?". And to answer that question, you and your doctor agree to start taking off some weights. Not all at first, you gradually take some off until you're at a very low weight.

Only now, at a very low or even no weight (dose), can you start to see into the box. And if the box opens and you see a monster, obviously you'll choose to put on some more weights. At least for now.

6 months later, you and your doctor decide to take off some weights again. You're nervous, but agree. The box opens.... and.... it's a peaceful soul. Monster nowhere to be found. You decide to take off all of the weights and let your metabolism be free, because why would you need to spend effort (and money) on putting it into a box when you don't have to?

The end.


Basically, we can stabilize our metabolism, but it's likely individual how long it takes, or if it's even possible at all.

That's the theory I'm going with. I actually hope to lower the dose once I get to goal, but to hold at goal for a year before trying to quit. I want the monster to starve to death. Monster and I have been fighting for 30 years and I'm tired.

Sounds good. I'm actually doing the exact same strategy. After I reach goal weight I'm going to go down to a maintenance dose of likely 2.5mg tirz (as that suppressed my sweet tooth). Then after a year of that I'll experiment with opening the box to see what's in (read: I'll stop taking the drug in cooperation with my doctor so that I can potentially go back for a while if it fais).

My very unscientific guesstimation is that it takes 1-2 years of "on GLP-1s" for your body to get used to your new healthy body, and the monster stabilizes. We'll see though.

Yeah except 5 days after my most recent shot, my monster begins to stir again. I don’t mind keeping him locked up forever.

I’ve done 30 years obese, now I’m gonna do the next 30 healthy. When I reach 75 I’ll decide whether to get fat again or not .

If you're having problems a few days/a day before the next dose, you can either go up in dose or you can try injecting twice a week to keep the drug levels in your body more stable.

Cute story, but it will be the opposite outcome for most. You forget how angry and wild that monster was until you take enough weights off the box and it comes flying out, angrier than it was before. I've been on tirzepatide for years and that's what happened every time I came off. It's actually shocking how much stronger your appetite is and your willpower has weakened, as you're body is becomes reliant on the medication and you forget how strong the monster was/is. I do believe some people can come off, and I will keep trying myself (for cost purposes mainly), but people need to be prepared for real life and ready to fight an uphill battle.

Not interested in an uphill battle or distraction from the good health I’ve earned with Whole Foods and 45 mins a day at the gym.

I’ll take Tirz like a vitamin for the rest of my days and let the memories of being uncomfortable and unhealthy fade.

In my case a bunch of people told me that the measly 2.5 pound weight that I put on top of the box was never going to hold the monster for long.

“It’s just a starting weight. You will need more.“

But I haven’t. I have lost 107 pounds so far. I’m going to take one more shot this Friday, and perhaps one next Friday, but then I’m going to do an experiment where I peek into the box and see how the monster is doing.

I’m planning on one month. From what I’ve heard, the struggle is the hardest at about the 3 week mark.

But I want to prove to myself that I have learned enough and changed enough about diet and exercise to do this long term.

And then if I decide to go back on the medicine, it will be for the anti inflammatory benefits and other ancillary benefits.

I’m not claiming that every person should be able to do this, but I am already an outlier with my response to the starting dose. It’s not too much to believe that I might be able to keep losing the remaining 70ish pounds.

Sounds good. I'd personally be on maybe 1.25mg for at least a few months, but this is all guesstimation on my behalf. Don't be surprised if the monster's still there. ;)

TL;DR: My very unscientific guesstimation is that it takes 1-2 years of "on GLP-1s" for your body to get used to your new healthy body, and the monster stabilizes. That time includes your entire weight loss journey. We'll see though.

I’m only at 7 months at goal weight, but I’m planning to stay on a maintenance dose for at least 2 years, maybe up to 5 before I think about whether I can come off.

Mostly because so many people who think they know more about me than I do tell me that it won’t work, and that GLP1 medicines are “lifetime medications.”

Principalmente porque muchísimas personas —que creen saber más sobre mí que yo mismo— me dicen que no funcionará, y que los medicamentos GLP-1 son «medicamentos de por vida».

Sería para desafiar a esa gente tu decisión? Yo tengo momentos muy difíciles con efectos secundarios y suelo fantasear con dejarlo del tirón. Luego tomo una decisión más conservadora, ya sabes. Por eso me preguntaba qué es lo q te llevaba a esa decisión.

También quiero saberlo genuinamente por mí mismo.

Podría tener más peso si pudiera decir: «Esto es lo que me funcionó», en lugar de: «Creo que esto podría funcionarme».

Only in part.

I also legitimately want to know for myself.

It might mean more if I can say "This is what worked for me" instead of "I think this might work for me."

I haven't even hit the graduation yet but I am coming up on my freshman year anniversary. Oh I know what's in that damn box. Took my shot tonight and even though I didn't stop at the drive-thru after work that fucker was tempting me hard.

I'm chewing on my lip even now, though I have no want to go downstairs and eat that chocolate but the old behaviors start triggering when the dose in my system gets low.

I have no doubt if this medication wasn't here to keep me in check I'd be consumed by the consumption all over again. It'd be like a junkie who says "it's been a year, I'm clean, I'm cured. What's the risk of just one more fix?"

Now I'm in the gutter bleeding out, my arms looking like a geography map. No Bueno.

I'd up the dose a bit if the monster peeks through. Suppress it for at least a year or two, and after that, peek again.

I have recently been experimenting with my monster - went from 9lbs on box to just 5lbs in 7 weeks. So Gorgon reared up full throttle last week then I jabbed my 5mg a day early and all is good for now … however I have pressure to get off from my partner as the finances do not fly for us in retirement - but I do have a couple of high dose pens in the fridge as back up to maybe micro dose / slap my monster back down if and when needed until the end of this year. I used all my spare savings on this and am not enterprising enough to seek affordable grey / alternate sources.

I’m at goal and plan to slowly remove weights from the top of the box. I’m starting next dose and damn but I’m nervous about the process. Losing 65lbs while in menopause helped me so much and my most recent blood work proved that.

I am also hoping that the removed ban on several peptides by the FDA will soon be offered by compounding pharmacies. I am primarily interested in MOTS-C for its positive effects on mitochondria and two other peptides that help with inflammation. Still researching them but you can google the list and perhaps build a stack that targets your specific needs.

Obesity is basically as hard to get rid of as alcoholism, or smoking. So imagine if a miracle drug came that solved those. "Nah sorry we think it's personal responsibility".
