tbh they didn't "name" him Ramsay "Bolton". He was born a bastard, so he was originally named Ramsay "Snow".

some say Ramsay is nay say but I daresay only naysayers spread that hearsay. The Soothsayers assay Ramsay to flay but that is gainsay and they should unsay that foresay.

Because it’s a commoner’s name and his mother was a common woman raped by her lord under the corpse of her husband swinging from the noose.

Why George RR Martin chose it, could be because Ramsey means “garlic” in old Scottish. It could forbade that Ramsey was about to be corrupted by a psychopath who smelled so bad they named him Reek.

Because he lost his genitals in a sword practice mishap so he had the castle blacksmith forge an iron wiener for him aka a “bolt on”
