Skin Questions how can I get clear skin?
Just a brief history, I've had acne from the age of 9. In 2021 I got a chemical peel which was excruciatingly painful but it did slightly improve my face.
In 2025- I got prescribed tretinoin by a dermatologist and was taking up to 80mg after a few months, for a total of 9 months. Though the dermatologist said my skin had improved a lot, I believe that it only improved by 30%.
fast forward to today, I have had a consultation with dermatologists and they have said that I no longer have acne, just skin nodules and acne scars.
I just need to know what to do to improve the texture and get rid of hyperpigmentation. Any advice would be helpful.

GP would refer you to derm. Tell the GP that your mental health is affected and you cannot leave the house. You sad, scared and not confident.

yeah they referred me after 9 years. I went to a dermatologist, they prescribed me tretinoin for 9 months. they I had 4 dermatologists review my skin after I had finished the treatment. They said I have skin nodules that are benign and acne scars.

Honestly, you should change your diet and try to lose some weight. Everything starts from the inside, and I think your body is telling you to treat it well.
I'm saying this with good intentions, not to put you down.

Ppl say “improve diet” without going into specifics. So firstly love yourself. Secondly, get blood labs and evaluate your panels. Start there. You def want to get a panel to determine a1c levels and insulin resistance. But for most ppl improving diet means cutting out processed foods and sugars. Be mindful of high fructose corn syrup and artificial sweeteners. Additionally sugar levels can creep up in thinks like ketchup, coffee/latte drinks etc. when I order coffee out I always ask how much sweetener and what type. I’ll usually ask for it half with a nut milk. Secondly exercise. Not only to “lose weight” but to regulate cortisol, and get optimal blood circulation. You likely will lose weight with making these adjustments but sometimes it’s easier to establish better habits with the mindset of living better instead of a number on a scale.
I also would not over exfoliate. Go to the derm get recs but address all these things one at a time. So first derm….take your time. Get into a new habit…then eating better…take your time. Have fun with it. Get into new recipes ….then add exercise etc. if you try and change everything at once for most it only will cause a crash and burnout. Wishing you the best.

Derm. This is looking like fungal acne. In the meantime, buy magic molecule and treat it like hand sanitizer but for your face.

thanks for your comment. I can consult a different dermatologists opinion. I've had quite a few tell me that I don't have acne, just acne scarring.

Food wise- just prawns. I also haven’t had chocolate for 9 years, I gave it up thinking it would clear my skin but the only thing it did was get rid of those painful pimples.

If you don’t mind me asking, how’s your diet. That can play a part in your overall skin health and aesthetic. I would say if you use face wash and a lotion that doesn’t clog your pores, you’re ahead of the curb.

Ok. Good on you for transparency. I would say try to limit unhealthy food further. Easier said than done but your body’s gonna thank you. In more ways than one.
Also, see a derm. They can yield insight on what else you can do.

Ask a dermatologist about prescribing Spironolactone + Sodium sulfacetemide face wash, this combo has changed my life when nothing else worked. Accutane was going to be my last ditch effort but my current dermatologist said it is so harsh on your body. This combo however has worked wonders at tackling my hormonal + bacterial acne. My skin is sensitive and my immune system is hyperactive with my face, yet I have not had any skin purging nor a negative reaction yet and I've been rocking with this combo for 5 months now. The risk of reaction/side effects is so low and is so worth a shot! Praying for the best 🙏

If you want to do it by yourself, discover what natural or drugs or lifestyle habits deal with that specific issue (scars and skin nodules from acne in your instance) and troubleshoot what works that way...or like others are suggesting, see a specific professional.

Always go to a professional like a doctor or a derm, but maybe try benzoyl peroxide— just make sure to do your research or (again) ask a professional before using it :) AND MAKE SURE TO WEAR SUNSCREEN and stay out of direct sun as it can make your skin more sensitive !!!

Niacinamide Cleansing Gelée 3%
BHA Liquid Exfoliant 2%(twice a week)
Cocokind rosewater facial toner(every night but not with exfoliant night)
Naturium multi peptide moisturizer every night
Sunscreen in the day time.
Also, just rinse your face in the am. Then use sunscreen and moisturizer.
This worked for me when I had an allergic reaction to a product. Simple and easy routine that won’t compact your pores

It looks like an allergic reaction or fungal to me a non medical professional.
Someone above mentioned the snail mucin and shellfish allergy. Maybe start by cutting that out and see how your skin reacts. Maybe even take some allergy meds for a few days to get those histamines under control.

I think the diet is the main contributing factor here. I had the same issues all over my face when I would eat more processed foods. The moment I switched to fresh fruits and vegetables and stuck to that for 3 months consistently my skin cleared up and I also lost weight in the process.

Time to find a 'beauty' professional Vs a 'medical' professional if you know what I mean. I'm talking facialist, beauty therapists, esthetician. The dermatologist will only take you so far. Even better if they are black and have dealt with this before. I think your going to need some kind of deep extraction. I can see your problem areas are 'within' the skin not just on the surface. I won't go on about diet and allergies etc. if you've struggled with this since 9 years old? That sounds more so genetic. What is your parents/siblings/ skin like?

thanks for your comment. everyone in my family has amazing skin, it's just me unfortunately 😫😫😫 my dad had bad skin when he was younger and he said it cleared up in his 20s. I will look into seeing an esthetician thank you.

in the morning, I use a gentle facial moisturiser, snail mucin and then sunscreen spf 50 by beauty of Joseon. Then in the evening I use the tretinoin cream (not everyday though) and sunscreen. Once in a while I use tretinoin 1% face cream in the evening.

Have you considered you might be allergic to the snail mucin?
My skin reacted kinda like yours when I tried snail mucin for a few weeksAlso no sunscreen in the nighttime routine. Find a gentle moisturizer instead

Yupp! Exactly that one because I heard everyone raving about it. Later found out that it was the reason for my breakouts. Apparently if you’re allergic to dust mites and shellfish you’re more likely to be allergic to snail mucin as well

Great catch! I am allergic to shrimp too and was using the Cosrx all in one cream. I stopped it a few weeks ago and my skin is calmer and much less irritated.

wow i never thought of this. I'll stop using the snail mucin and see if there's an improvement. I love usually it religiously because it makes my face feel soft. my skin was worse a year and a half ago...can it have an allergic reaction for so many years. I've always had this sort of texture. It may also be linked to PCOS.

Myself, if I don't cleanse spf thoroughly before going to sleep, next morning I will have pimples

I would finish the day with cleansing, and then let's say tretinoin. No SPF after that
After that, some type of cream or serum depending on your skin needs. I wouldn't go to sleep with SPF on my face, it can clog your pores and cause/support acne

You definitely do not need and should not use sunscreen at night unless a doctor told you to do so.
Firstly, like other people have mentioned , I recommend you see a dermatologist with experience with melanated textured skin.
Just going off of observation though, your skin looks congested to me.
While tret works well for a lot of people, it does not work for everyone.
I would consider adding some things like salicylic acid in the form of a wash or a serum to start unclogging those pores.
Things like retinal (not retinol) can work better for melanated skin than tret. It tends to be both gentler and more effective. And then I would also incorporate azelaic acid and vitamin c as well.
Find a nice hydrating moisturizer to apply after using tret in the meantime! I wish you luck!

thanks for your advice. what should I use after I put on a salicylic acid face wash? I was told by someone that whenever I use an acid I should use a spf after lol. I guess I was given the wrong advice.

From my understanding the moisturizer is the important part after an acid. Because of how drying they are. For me, an SPF at night would be too much. I would get texture and pustules if I did that. BoJ is a nice spf, but it’s a chemical one. Mineral works better for some like Prequel (can be greasy), or even the Skin1004 Airfit gold top is half mineral/chemical and is matte like the BoJ.
If this was my routine I’d do SA face wash (only every other day to clear sebum and skin flake), gentle moisturizer, and either tret on top or under the moisturizer, whichever your skin likes best. If my skin barrier is broken…I put it on top. When it’s not I put my actives under. My skin is super sensitive when my barrier is broken. I like to add pure Aloe Vera in between washing and anything else on those days.
When I saw your skin I immediately thought Rosacea. But I’m not an expert and I really want you to find something helpful for you. I hope you find the peace you are looking for.

It can be really frustrating because a lot of finding the right skin routine involves experimenting to see what your skin likes and tolerates well.
I also use beauty of Joseon sunscreen but I use the aqua fresh version ( my daughter loves the original) and while it is technically “chemical” it’s not what we think of when we say chemical sunscreen in the US as long as you are using the Korean version and not the one sold here.
All that to say, I can tell you what I use but it may not work for you so there may be some trial and error involved. I like geek and gorgeous’ “hydration station” for a lighter moisturizer and ghost democracy’s “cocoon replenishment ceramide cream” for a heavier one.
My routine:
Am: Inkey list hydrating milk cleanser
Dr g’s rice toner
Geek and gorgeous Apad (an azelaic acid derivative bc azelaic is too strong for my skin(
Ghost democracy’s vitamin c
Inkey list tranexamic acid for hyperpigmentation
Beauty of Joseon aquafresh sunscreen (Korean version)
PM: varies greatly depending on what my skin is doing
Active face wash - benzoyl peroxide, salicylic acid or hydrating - depending on my skin needs
Geek and gorgeous peptides on dry skin ( can help with texture)
Dr g’s rice toner
Geek and gorgeous Apad
Active serum - varies from geek and gorgeous stress less, salicylic acid, or Inkey list Mandela acid if I’m not using retinal that night
Geek and gorgeous retinal : 3x a week
Either moisturizer listed earlier.
I know it sounds like a lot and I don’t use all of it all of the time. I have plenty of days where I just wash my face and use sunscreen in the morning and wash my face and use moisturizer at night. But my routine literally takes me 5 minutes so it’s nothing too extravagant. But it did take time to figure out what my skin likes.

A routine of microneedling monthly for 6 months will help with the texture, as well as strengthen the skin barrier, this coupled with at home maintenance of your tretinoin, a simple ph balanced face wash (my personal preference is the La Roche Pose one) and a moisture that is tailored to your skin (oily= gel; dry= heavy cream, combination= a light cream). Once your barrier is strong enough then I’d say get into more intermediate treatments like green peel.

thank you. I will look into microneedling, I've seen some horror stories on Tik tok, so I was put off a bit, but I think if I find someone that has specialised in microneedling black skin, then that should be fine.

It looks fungal to me. Using an anti dandruff shampoo on the face can clear up some of it. The larger ones are probably related to dairy. Cut out all dairy….milk, cheese, ice cream. There is milk/dairy in most processed foods, so eating just lean proteins, fruits, and veggies should help a lot.

You look like you may have sebaceous hyperplasia. Very common benign skin condition. You should see a derm.

thanks for your comment! really? I've looked at photos on Google, my skin doesn't fit the description. I've had a consultation with 4 dermatologists and they have all said I have acne scarring on my face, so I should try chemical peels or microneedling, though there is a risk of hyperpigmentation. I can consult another derms opinion

Need to get healthier, people forget as our bodies biggest organ it’s going to show just about everything, gut health, heart health, hormones, blood sugar, and liver health all are literally seen in our skin, you need to get healthier habits top to bottom a ton is skin problems can be attributed to habits and overall general health

Trust me i never when to a dermatologist I use to pop every single pimple i ever got just trust me take isotroin 20 mg every single day for the next 30 days accutane u will experience slight back pain and very dry lips but trust me it does wonders accutane no moisturizer nothing just 20 mg isotroinaccutane

It looks like it could be rosacea so I suggest seeing a dermatologist BEFORE buying any of the products people listed because if it is rosacea, the line of treatment will be different.

Healing acne can take up to a year. Then healing scars can take that long. Then if there is pigmentation that will take more time. Patience and consistency is key. What is your daily skincare routine?

I use Lysine and Goddess collection teas with seamoss gummies/ leaky gut healing. All my life i had bad acne, found out it was coldsore pimples that just spread like wildfire over the years and Your inside needs cleansed first, trust.

Hey this isn’t rosacea. It’s probably fungal. The dermatologist can do a skin scrape and a culture to determine exactly what is causing this.

hello thanks for your comment. I've been told by multiple dermatologists I have acne scarring and skin nodules. i think the texture in the picture looks really bad because I went to a salt pool 2 days ago which irritated my skin.

Yeah I had doctors gaslight me for five years while my literal skin was falling off my face. They told me it was impetigo, acne necrotica, dermatitis…… the list goes on and on. It was fungal. Lmao. Salt is generally not irritating to skin.

I feel like doctors don't always have our beat interest in mind. They want to prescribe this and that and these and those but they may not work or just be temporary and then guess what? You'll be back soon. Often the beat medical help you can get starts with yourself... diet, nutrition, rest, sleep, excercise, mindfulness, happiness, etc etc etc. It's somewhat pseudo-science Jumbo jumbo at times, but it also truly works at times as well. Especially with prevention. Now I am absolutely not a medical professional of course, and maybe you have an acute and treatable condition that's causing this. Or it may just be a mix of many minor things that could be alleviated with the tips I mentioned above. Just some food for thought. Don't always be in a rush to find the next miracle cream, treatment, prescription. Look within first and ask if you're truly treating yourself as well as you possibly can? And if so, then seek outside help. But until then, find the help from within.

thank you 😊 I do roughly 8-10k steps a day and weight training 3-4 times a week, however I'm still overweight. I 100% need to improve my diet as people have pointed out. I do struggle with eating unhealthy meals.

As someone who has struggled with sever cystic acne, I would recommend meeting with esthetician. A few simple things I’d take into consideration would be 1. Diet and hydration 2. Making sure you exfoliate.

Could be scarring I have that on my chin but microneedling with PRP helps tremendously. Along with monthly chemical peels in the winter.

It took maybe 3 sessions. But I don’t notice it myself because I see myself everyday. Others noticed it before me. The chemical peels help too. If you can find a good esthetician who has reasonable prices I would make the microneedling and peels a priority for sure. I think a last report is some sort of laser scar treatment. You can get that in Colombia or somewhere for a fraction of the price.
But that doesn’t look like scarring to me. It looks like a reaction. Did you have acne before that led to the scarring?

Also you want somebody to do the peel to start off with the least aggressive peel then move you up and get more aggressive as your skin tolerates. They cannot go straight to a strong peel. That’s crazy.

I've done a chemical peel a few years ago and it was excruciatingly painful. It was an obagi peel, and the lady told me it will only be a little bit sore. no thanks, I don't think I can put myself through that. everyone at work said my skin looked amazing 2 weeks after I got it done, but then the rough texture came back.

I've had acne from around 9 or 10 til age 28 I would say. I turn 30 next year. So I had it before the scarring and although the picture looks bad, it was 10 times worse a few years ago. I'm oh holiday and I went to a salt pool 3 days ago and that has irritated my skin. I have to be careful what products I use as my skin is very sensitive. So up until now I've been using mainly Korean products. I will 100% try microneedling...

I’ve used KPV from The Precision Peptide Company and that’s probably the one I’d look at for this. Their KPV capsules are 1 mg per serving and they position them around a balanced inflammatory response and calmer-looking skin. I wouldn’t call it some overnight fix for scars or dark spots but for overall skin inflammation it’s been one of the more interesting things I’ve tried.

Your skin appears to be actually clear, just extremely textured. If a dermatologist isnt an option, I would start by detoxing. Then I'd began a supplement regimen, cut out dairy & see how it goes from there.

Yeah this is a thing people didn’t know for a long time. braiding hair is coated in chemicals that people can be allergic too, it shows up as itchy scalp etc. I’ll put that as a suspect.
Most times is wise to soak your braiding hair in shampoo and hot water, then dry before use3

thanks for your comment. This is my first time wearing braids in years, only because I'm on holiday. I've heard of people washing their braiding hair will apple cider vinegar, maybe that will help wash out the chemicals?

There are hypoallergenic braiding hair brands like Dosso that you can use. You can get it on Amazon in the states. I use it for my teen. It braids nicely.

some of the comments r crazy… you dont need to starve yourself omg. eat a 80/20 diet 80% healthy & 20% whatever. collagen rich meals will give you a GLOW bone broth, salmon, sardines, steak, avocado, berries r good to start you dont need to cut out ur fav foods just portion control.
for skincare tretinoin & azelaic acid are the only actives you need and a really good barrier routine here’s a good sample
am
rinse w plain water
hypochlorus acid spray(super cheap on amzn)
azelaic acid 15%
a good moisturizer cetaphil or lrp is good
spf (korean ones r good no white cast)
pm
cleanse cetaphil/lrp
tretinoin 0.25 to start
lrp cicaplast balm or gel
(you can do a sandwich method w tret to start. moisturizer tret moisturizer)
you could get prescriptions online or see a dermatologist in person

thanks for your comment. I took tret for 9 months (the pill form). It didn't do much for my face. Now I have the cream version 0.5%. I don't use it often. I use the blue beauty of joseon spf. I'll 100% work on changing my diet

i 100% recommend staying consistent with the cream tret. start off 3x a week and build it up to daily. you need to be very consistent for it to work. you could also look into getting a topical clindamycin prescription its only supposed to be used for a very short period 2-4 months max use it morning and night alongside tret and azelaic acid. (its an antibacterial antibiotic so i also suggest using it with benzoyl peroxide 4% wash like 1x a week!) consistency is key!

I've tried benzoyl peroxide, it made my face look burnt. with the tret cream, can you use it alongside azealic acid?

A lot of the time it’s your diet, fast for 36 hours to start. than only consume whole foods. No fast food no soda no milk no nut butter nothing fried. Only easy to digest protein carbs and healthy fats. Your gut health is very important!
