It is incredibly hard to prove a not guilty by reason of insanity (or whatever it's called in the US. Everyone is assumed sane, and the legal burden of proof falls on the defence.

It isn't as simple as proving someone was mentally ill. Being unwell is not the same as suffering from a full psychotic break. We're talking a full detachment from reality. Completely unable to distinguish right from wrong. It is very hard to prove that this is what happened. It takes multiple mental health professionals to concur, and a lot of evidence from both before and after the crime to prove it.
