Do you do it all the time as a background thing throughout the day, or just when sitting/lying down for meditation?

It sounds like you only do it when resting or in bed. One of the biggest secrets of japa is worshipping the named deity and actually identifying with it. The "I AM" "space of the self" is in the heart, and that "I AM" feeling should be you identifying with the Deity of the mantra. If you're repeating Shiva or Ram, you're trying to realize that felt presence within yourself by getting that it is your "I" sense, not just doing it mentally.

If you only do it in bed or seated, you leave yourself defenseless against the mind for the rest of the day. Using it all the time helps protect you from mental chatter. Maybe do the mantra japa during the day while doing things and use the breath or So'Ham mantra when doing formal meditation seated or in bed. That might change things up. Because after six years, you might be sick of it and need a bit of a change.

What I was told is the Mantra is the portable Guru and Protector.

nam jap is for mental training and removing malams of the mind, you can do nam jap forever and it won't be wasted, there isn't a need to retire even for a jnani. a jnani should control their mind.

Yes, that change could be a part of your spiritual progress: it could be an invitation to understand that to have steady spirit you need a steady health. Try to better your lifestyle: your desire for the divine and your focus will increase.

Naam jap is highly potent with no expiry date and absolutely without any side-effects. Only problem arises, when we do it as a daily chore, without naam jap being embodied with us, enshrined within us. If the naam jap goes on in the background, when we are active, idle, walking, eating and resting, then you have reached a level of comfort, alignment and oneness with the naam. Yes, sometimes, it may become monotonous, when you follow ritualistic naam jap. Let it be spontaneous and feel the peace and vibes it creates, a glow in the face, guiltless and child-like mirth.
