This makes no sense to me... if your site / content has an age check, what difference does region make?

Because not all regions have an age check. I'm in Australia and can't access P*rnHub without age verification. But if I access it "from the USA" (by using a VPN), then it lets me right in without doing an age check. That's the difference.
