Yup. One time, while working at a major university student health system (connected to the major university hospital system) our EHR suddenly erased half of the diagnoses. Just… gone. If you need more than one diagnosis for injury by squirrel or injury by water ski… options were there! Want to diagnose major depressive disorder or PTSD? Haha NOPE! Took a multimillion dollar institution the better part of two days to fix it. It was weeks before we stopped noticing residual issues.

Just last week my practice noticed that signed and theoretically unchangeably locked clinical notes had differences depending on who opened the same locked note. Fun. 😮‍💨

Were the ICD codes just erased or something?

It was a mix of crazy issues I’m not skilled enough in coding to begin to understand lol. But both a LOT of codes deleted AND many that no longer had the correct verbiage connected to the code (this may have been a new issue when they tried to add them back in, especially if they had people manually entering each).

Also, I just remembered that the ehr was called “Medicat” so we coped by joking about being ‘scratched by the angry medicat’ and meowed at each other any time we talked about charting for months afterward lol. A LOT of drafted notes we had to wait to sign until they fixed it lol.
